The System Health Check portal was deactivated in 2011. However, some older paper forms may still include instructions to submit samples online.
A few retailers may still have stock of these test kits including the old forms, so the information can vary. Please note that the process is entirely paper-based, and no online submission is required before posting your System Health Check sample.
